Private Duty Nursing

Providing premier non-medical attendant care to assist older adults and the chronically ill with their activities of daily living. We are able to provide care up to 24 hours a day, on a temporary or long-term basis, in your own home. Certified home health aides and home care aides provide assistance with personal care, companionship and home management on an hourly or live-in basis. LPN and RN services are also available. All caregivers are PCA-level trained or higher, screened, bonded, insured and covered for Worker's Compensation, allowing you to have peace of mind with the individuals being brought into your home. Before service begins, you will meet with a professional nurse to determine the care needed and develop a plan of care.

Traumatic Brain Injury Waiver Program

Traumatic Brain Injury Waiver Program (TBI) is a New York State Medicaid program that provides services to persons with a TBI. The purpose of the program is to help persons with a TBI live in the community setting of their choice. Medicaid funds the program. Services included in the TBI Waiver are Service Coordination, Home and Community Support Services , Independent Living Skills Training, Structured Day Programs, Substance Abuse Programs, Positive Behavioral Interventions and Supports, Community Integration Counseling, Environmental Modifications, Respite Care, Assistive Technology (special medical equipment and supplies), Waiver Transportation, and Community Transitional Services.

The Nursing Home Transition and Diversion Waiver Program (NHTD) is a New York State Medicaid program that offers individuals with disabilities and seniors citizens who are eligible for nursing home placement to remain in or return to the community. The program is designed to help individuals have more input and decision making in their care. Services offered through the waiver include Service Coordination, Home and Community Support Services, Independent Living Skills Training, Structured Day Programs, Moving Assistance, Respiratory Therapy, Home Visits by Medical Personnel, Positive Behavioral Interventions and Supports, Wellness Counseling, Community Integration Counseling, Environmental Modifications, Respite Care, Assistive Technology (special medical equipment and supplies), Peer Mentoring, Nutrition Counseling, Congregate and Home Delivered Meals, and Community Transitional Services.
