  • Homemaker & Companion Care
    Our Homemaker & Companion Care services assist with light homemaking, laundry, meal preparation, shopping and providing a watchful eye regarding clients safety. This service may also include out-of-home accompaniment, serving as a guide and companion.
    Home Health Care Aid Services
    Home Health Care Aid services assist elderly, convalescent or disabled persons living in home, provide health-related services under direction of nursing or medical staff such as medication reminders, light prescribed exercises; keeping patients rooms clean and personal hygiene. A great option for clients recently discharged from the hospital that need assistance transitioning back to their home.
    Traumatic Brain Injury Waiver Program
    Traumatic Brain Injury Waiver Program (TBI) is a New York State Medicaid program that provides services to persons with a TBI. The purpose of the program is to help persons with a TBI live in the community setting of their choice. Medicaid funds the program. Services included in the TBI Waiver are Service Coordination, Home and Community Support Services , Independent Living Skills Training, Structured Day Programs, Substance Abuse Programs, Positive Behavioral Interventions and Supports, Community Integration Counseling, Environmental Modifications, Respite Care, Assistive Technology (special medical equipment and supplies), Waiver Transportation, and Community Transitional Services.
    The Nursing Home Transition and Diversion Waiver Program
    The Nursing Home Transition and Diversion Waiver Program (NHTD) is a New York State Medicaid program that offers individuals with disabilities and seniors citizens who are eligible for nursing home placement to remain in or return to the community. The program is designed to help individuals have more input and decision making in their care. Services offered through the waiver include Service Coordination, Home and Community Support Services, Independent Living Skills Training, Structured Day Programs, Moving Assistance, Respiratory Therapy, Home Visits by Medical Personnel, Positive Behavioral Interventions and Supports, Wellness Counseling, Community Integration Counseling, Environmental Modifications, Respite Care, Assistive Technology (special medical equipment and supplies), Peer Mentoring, Nutrition Counseling, Congregate and Home Delivered Meals, and Community Transitional Services.
  • Personal Care
    Personal Care for bathing, grooming, hygiene, transferring, toileting, and other needs.
    Dementia Care
    Dementia Care provided by our specially trained caregivers.

    Private Duty Nursing
    Providing premier non-medical attendant care to assist older adults and the chronically ill with their activities of daily living. We are able to provide care up to 24 hours a day, on a temporary or long-term basis, in your own home. Certified home health aides and home care aides provide assistance with personal care, companionship and home management on an hourly or live-in basis. LPN and RN services are also available. All caregivers are PCA-level trained or higher, screened, bonded, insured and covered for Worker's Compensation, allowing you to have peace of mind with the individuals being brought into your home. Before service begins, you will meet with a professional nurse to determine the care needed and develop a plan of care.
    Nursing Care
    Nursing Care tailored to clients medical needs in consultation with physician or managing agency such as Long Term Care Program, Hospice or Hospital.
    Respite Care
    Respite Care to give family caregivers needed relief.
    Medication Set-Up
    Our Medication Set-Up Service, performed by a RN or RN-Supervised LPN is done on a weekly or bi-weekly basis (for no more than a 15-day supply, in accordance with NYS regulations). All medications are reviewed and distributed into medication boxes or single use containers.
    Consumer Directed Services
    This Medicaid program provides services to chronically ill or physically disabled individuals who have a medical need for help with activities of daily living (ADLs) or skilled nursing services. Services can include any of the services provided by a personal care aide (home attendant), home health aide, or nurse. Recipients have flexibility and freedom in choosing their caregivers. In addition, the consumer or the person acting on the consumer's behalf (such as the parent of a disabled or chronically ill child) assumes full responsibility for hiring, training, supervising, and, if need be, terminating the employment of persons providing the services.
